Do I have to get the MMR vaccine?
Yes! It is required for all incoming students to submit proof of immunity against Measles, Mumps and Rubella (MMR), regardless of how you will be attending classes (in-person, hybrid or online). Failure to provide proof of immunity may prevent you from enrolling or being dropped from your classes. The MMR vaccine is normally given to you as a child, so you may have already received the vaccine.
Can I receive vaccination at the student health office?
Yes! We now have a full-time nurse that is able to provide you with the MMR vaccine. However, we do not provide students with the Meningitis vaccine as it is optional. If you would like to receive the Meningitis vaccine you may visit a CityMd, pharmacy (CVS or Rite Aid), urgent care or speak with your doctor/health care provider for vaccination.
If I am completing the form, does the doctor need to sign the form?
If you are completing the form, you do not need to have your doctor sign the form. You will just need to submit your vaccination records for proof of immunity. When submitting your vaccination records, please note the following:
- Be sure that your name and date of birth are some where on your records (full name and date of birth mm/dd/yyyy)
- If you are submitting an image of your vaccination booklet be sure to send a picture of the front and the back of the booklet.
- If your records are in a language other than English, please have them officially translated before sending them to us. When submitting your records online, please make sure that the image is clear! If the image is blurry or unclear, we may ask for additional proof of vaccination.
What if my vaccination records are in another language (not English) can they still serve as proof of vaccination?
You may have your records officially translated into English and submit them with your Immunization Record Form, or you may have your provider complete the form. If the provider is completing the form, they must stamp the form with an official stamp that includes physicians name and license number. If the form is filled out by the provider and not stamped, we can not accept the form. Stamp must also be in English.
What if the name on my vaccination records does not match my CUNYFirst account?
If your legal name is different from CUNYFirst, please provide proof of your name change. (marriage certificate/ legal name change documents) and state ID or passport so that we may verify that those are your records.
What if you are unable to access your records/meet with a doctor, but believe you may have been vaccinated?
You may request a titre test from your physician. A titres test will sohw your immunity to the MMR (Measles, Mumps and Rubella) diseases. Once you do the test, you will mark the box on the immunization form and input the day you took the test. You will then submit your lab results and the Immunization Record Form to Student Health via the secure transfer file drop. Please be sure that your lab results contain your full name, date of birth and numerical range and result on the lab report. If there is no numerical range, we are unable to verify if you meet immunity. Equivocal results will not be accepted. Please note: All three values must be in the positive range in order to be accepted. Depending on your level of immunity, you may require 1 or 2 additional doses of the MMR vaccine.
What if I can't get vaccinated because of religious or medical reasons?
If you are unable to receive the MMR vaccine due to religious or medical reasons, please email us at shs@ccny.cuny.edu and we will inform you how to proceed.
Where can I find my vaccination records?
In New York State, vaccinated children are given a yellow immunization record booklet. Typically, the first dose of the MMR vaccine is given at 12 months old. If you have access to a vaccination booklet or records of when you were a kid, you may find the MMR vaccine on there. If you have access to your records online, such as a Mychart, you may provide us with a screenshot of your records for proof of vaccination.
Any records submitted must contain your full name and date of birth!
What if I don't have an EMPLID?
If you are an enrolling student (whether degree seeking or non-degree), you should have an EMPLID. You may find your ID by logging into CUNYFirst, click on 'Student Center' and under profile there will be an 8-digit number (EMPLID).

Where do I submit my records?
You may submit your records to us online or in-person. To submit online, please use our secure transfer link to upload your documents. If you would like to submit your records in-person you may stop by our office in Marshak room J-15. (We are in between the planetarium and gym!)

What forms do I need to complete?
If you have a 'MMR Immunization' and 'Meningitis Immunization' hold on your account, you will need to complete the Immunization Record Form. You will complete the student information completely (be sure to include your EMPLID!), part 1A (MMR) and part 2 (Meningitis).
If you only have a 'Meningitis Immunization' hold, you will only need to complete the Meningitis Record Form. You will complete the student information section and then mark one of the options that applies to you.
Once completed, records and forms may be uploaded to our secure transfer or handed in in person at our office in Marshak J-15.
