Make sure you are aware of all the enrollment requirements described on the previous page before completing the application form below. After completing the form, click submit only once (there will be a brief delay) and then a submission success webpage will load. Immediately after successfully submitting your application, you will receive a confirmation email (otherwise, check your spam folder). 

Applications will be reviewed once the application period ends. Unless there is a problem with your application, about a week before the first day of classes for the upcoming semester, you will be emailed about your application status and then enrolled. 

Your grade and continued CHIS enrollment is contingent upon your submission of a graded research paper and progress report to CHIS.
